Altered scapulothoracic mechanics could be due to poor strength, capsular tightness or other postural changes that increase the risk of injury. 90-degree push-up This test is part of the Presidential Youth Fitness Program and can be used by almost everyone. It's especially useful for preteen athletes when compared to pullups. 3 For athletes with a history of shoulder instability, I don't use this test. But for everyone else, it's an easy method to assess overall upper body strength. While untimed, I have found that most athletes in the youth and adolescent population are unable to perform proper pushups to the right cadence of one pushup every three seconds. It's important to pay special attention to the elbows and hips during this test, as they will be the common areas of compensation or failing to maintain proper technique. If the athlete can't perform multiple 90-degree pushups, identify the first signs of compensation. For instance, as fatigue sets in, the right hip may drop while the left is level, indicating rotational core weakness. This allows you to set a training program with greater levels of detail instead of basing it on the number of pushups performed. Lastly, check how well the athlete breathes during this test. Poor breathing mechanics can have significant ramifications in the overall picture. Sports medicine specialists in the school system often don't have the luxury of spare time throughout the day. It's imperative to develop a systematic, efficient and detailed group of tests that can help assess an athlete for these situations. These four tests have been studied and are easy for athletes to perform. Additional research could further improve on the interactions to other systems, quickly providing even greater levels of detail.
